Transcultural Team
Description of the service
The Transcultural Team works to improve access to mainstream mental health services for people from black and minority ethnic (BME) communities.
The Team work alongside colleagues in the Community Mental Health Teams (CMHT) to provide greater understanding of cultural issues and more culturally appropriate assessments and care plans for ethnic minority communities - this can be through consultation, advice and joint working.
The Team Workers also manage a caseload of service users. People are assessed and provided with a care plan that addresses specific mental health needs.
Referrals to the team are from CMHTs, other Statutory agencies, the Voluntary Sector and GPs. Appointment times and places are negotiated with individual service users.
Who's in the team?
There are 7 full-time equivalent Social Care posts (Mental Health Social Workers and Mental Health Workers) in the Team, representing six of the largest BME communities in Sheffield - African-Caribbean, Bangladeshi (½ post), Somali, Pakistani (2 ½ posts), Yemeni and Irish. These workers combine specific language and cultural expertise with mental health skills.
Where is the service located? The Service is City-wide. The office base is at East Glade Community Mental Health Centre.
How is the service accessed?
By referral - on the Sheffield Health and Social Care Referral form, by letter or by phone.
